If, for any reason you are unable to submit an order online, you can use this handy printable text form or pdf form to send us your order via Postal Mail. Please note: if we have not charged your credit card before, we must obtain a positive address verification. This means you must provide to us the EXACT address where you, the cardholder, receives your monthly billing statement and we must ship it to that address. Therefore, it is important you fill out the form completely and correctly to avoid delays in receiving your item(s).
